The 2026 Citizens Government Academy will meet on Thursday evenings from September 3 through November 5, 2026, from 6:00-8:00 p.m. at various Columbus Consolidated Government facilities. 

Photographs, video recordings, and audio recordings may be taken during Citizens Government Academy sessions. These materials may be used by the Columbus Consolidated Government for informational, educational, promotional, and public-information purposes, including social media, websites, newsletters, publications, presentations, and news releases. 

By signing below, I certify that the information provided is accurate and acknowledge that I have reviewed the program requirements, attendance expectations, and photography and media consent options. 

I,being 18 years of age or older, in consideration of my participation in the Citizens Government Academy conducted by the Columbus Consolidated Government and my being allowed to accompany members of the Columbus Consolidated Government in its buildings, properties, and vehicles during various activities with the Citizens Government Academy, agree as follows: 

a) I release the Columbus Consolidated Government from liability to myself and my heirs for any damage to my property, injury to myself, or loss of my life sustained as a result of my participation or my accompanying said persons, properties, or vehicles during Academy activities. 

b) I agree to hold the Columbus Consolidated Government harmless for any loss sustained by it as a result of any injury or damage caused by me, and I agree to indemnify the City, its agents, or employees for any loss incurred thereby. 

c) I consent to appropriate emergency treatment in the event of my illness or injury during my participation in the Academy, and I agree to assume responsibility for any costs incurred thereby.
