Requirements to Apply for a Passport

1. Completed DS-11 form in BLACK INK ONLY (form is available in office)
2. Proof of Citizenship (one of any of the below options)
   • Certified Birth Certificate (MUST have official seal and NOT a plain copy)
   • Naturalization Certificate
   • Previously issued Passport
3. Photo ID
   • Driver's License
   • State/Government Issued ID
4. One Passport photo^* (acceptable passport photos must meet stringent specifications)

^*photos can be taken in office for a $15 fee

• Payment #1:
  $35 Application fee paid to Probate Court - cash/cashier’s check/money order/credit or debit card ($2.50 processing fee)
• Payment #2:
  Check or Money Order ONLY payable to US Department of State^*.
  • Adult Passport Book: $130
  • Adult Passport Card: $30
  • Minor (under 16) Passport Book: $100
  • Minor (under 16) Passport Card: $15
  • Expedite Fee: $60
  • 1-2 Day Delivery Service: $22.05(effective 6/27/25)

^*If you are unsure on what fees will be owed, please call the office before purchasing a money order, otherwise bring a blank check.

Minor Applicants (under 16)

• Minor MUST be present
• Both parents MUST be present OR bring completed DS-3053 form (Notarized Statement of Consent) AND front and back copy of Driver’s License/ID from absent parent
