Passport Renewals (Issued at age 16 or older)

Passport renewal by mail using Form DS-82 is possible if the most recent passport meets the following requirements:

1. Issued within the last 15 years
2. The applicant was 16 or older at the time of issuance
3. The passport can be submitted with the renewal application
4. The passport is undamaged (other than normal "wear and tear")
5. The passport is issued under the current name or can be supported by a certified copy of proof of change, such as a Final Order or Marriage Certificate

If any of the above requirements are not met, a new application must be completed in person using form DS-11.

Minor (15 & Under)

New Applicants

• Child must be present for application processing
• If both parents are named on the birth certificate, both must be present. If one parent is unable to attend, a Consent Form (DS-3053) and a front/back copy of their GA State issued ID must be provided. f you are out of state, you must provide a secondary I.D., work school, or military I.D and the names must match. If there has been a name change, provide a certified copy of proof of change such as a Final Order or Marriage Certificate.
• Passport Application (Form DS-11)- Must be completed using BLACK INK ONLY
• Proof of U.S. Citizenship - U.S. Birth Certificate with a raised seal or an original Naturalization Certificate.
• Passport Photo
• Application Fee + Application Execution Fee + Passport Photo (if taken by this office)

Fees

• Passport Photo - $20.00 each (Cash, Money Order, Credit/Debit Card - Payable to Muscogee County Clerk of Superior Court)
• Application Execution Fee - $35.00 (Cash, Money Order, Credit/Debit Card - Payable to Muscogee County Clerk of Superior Court)
• Application Fee - see chart below (Check or Money Order - Payable to U.S. Department of State. Include the applicant's name and date of birth on the front of the check or money order. Each application must have a separate form of payment. Temporary or starter checks are not accepted.
